TITLE: LPN Nurse PAY GRADE: 48

JOB DESCRIPTION:

Provide support to learning by promoting the health, well being and safety of students and staff.
Administer health services to students and staff according to district policies; provide nursing care for ill,
medically fragile and/or injured students; serve as a health care resource to staff and administrators;
provide case management services; manage and conduct mandated hearing and vision screenings;
identify health problems referring to medical provider for follow up treatment as appropriate. Work
collaboratively with school and district staff and community professionals to meet health care needs of
student in school setting. Provide case management services as appropriate.

QUALIFICATIONS:
1. Minimum Educational requirements for LPN Licensure
2. Current Arizona Licensed Practical Nurse (LPN) License
3. Two (2) years of clinical experience (pediatrics preferred)
4. Valid Arizona driver’s license (required)
5. Registered as a Medicaid provider
6. Obtain and maintain CPR and First Aid Training certification
7. Obtain and maintain Nonviolent Crisis Intervention (NCI) certification
8. Knowledge of Immunization requirements according to Arizona State Law
9. Ability to respond quickly and appropriately to emergency situations
10. Ability to work autonomously, making sound decisions while under pressure in a variety of
situations.
11. Ability to communicate effectively (both verbally and in writing) with students, parents/guardians,
staff and outside agencies
12. Proficient in the use of a computer and basic software applications
13. Ability to maintain confidentiality of medical records and communications
14. Ability to lift and transfer students up to 50 lbs.

DUTIES AND RESPONSIBILITIES:
- Coordinate health services within the assigned school.
- Provide direct nursing care by administering first aid, emergency care, medication administration
and specialized medical procedures to ill and/or medically fragile students, and students with
mental health needs according to district policies and protocols.
- Assess student safety, abuse, health related issues, and identify problems, referring as
appropriate and complying with legal requirements. Document accurately.
- Identify, assess, plan, evaluate and document student health concerns.
- Follow Arizona State Laws, district policies and protocols related to communicable disease,
immunizations, mandated hearing and vision screening, and child abuse. Refer as appropriate
and submit required reports.

- Supervise and administer mandated vision and hearing screenings, referrals, and assistance with
follow-up care. Submit required reports.
- Perform procedures following district and ADE guidelines.
- Provide education and training related to acute and chronic illness and health related topics.
- Act as a resource for students, parents/guardians and staff.
- Develop individualized care plans and emergency plans for students with health care needs,
maintaining contact with staff, parents/guardians and/or medical providers to implement health
care directions and notify of changes in student health in the school setting.
- Participate in a variety of meetings (MET, IEP0 related to students.
- Attend staff meetings, health service meetings, staff development and other required meetings as
assigned.
- Maintain Medicaid documentation and submit monthly
- Participate in Random Moment Study.
- Provide first aid and health counseling for staff, documenting according to district policy.
- Maintain health office, supplies and equipment.
- May supervise and train health assistant(s).
- Perform other duties as assigned.

KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS:
? Knowledge of special education laws, procedures, practices, materials, equipment, and
compliance standards required to provide appropriate learning experiences for special education
students. Good knowledge of the nature and effects of the impairment(s) in all areas of disability
? Ability to establish and maintain effective working relationships with peers, parents and the
community; excellent human relations skills; skill in both oral and written communication; ability to
work in multicultural environment with diverse population of students, staff and parents
? Ability to control certain behavior(s) through physical restraint if necessary
? Knowledge and proficiency in district designated computer hardware and software including
Microsoft Office, MET, and IEP software programs and other relevant software
? Ability to coordinate activities with related and specialist staff
? Knowledge of the principles and practices of licensed practical nursing care
? Knowledge of district policies and procedures affecting assigned work
? Knowledge of state and agency laws and regulations governing general licensed practical nurse
practices
? Skill assisting students with medical conditions
? Skill assessing medical situations and determining course of action
? Ability to communicate orally and in written form
